IE 11 Browser: Table Tools button display proper.
Firefox 30.0 Browser: Table Tools button does not display proper except Print button.
Chrome Browser: Table Tools button display proper.

Works ok (all buttons) on Firefox 33 (Aurora)... maybe it is an issue with earlier firefox versions?

Yes. If you don't have Flash installed, then the Flash based buttons will not appear. This is likely why different people are getting different results.
