Add checkboxes and submit button to the table

Hi,

I want to add multiple chekboxes which is generated from php loop and a submit button to the table as table. Initially the submit button is disabled and if user click on any row the checkbox on that row is checked and submit button become active, finally the form is submit to the server, I have already implement the form but it is only working in the first pagination page. Please can anyone help
